Verona & District Medical Services Find-A-Doc Reward

Contest Rules and Regulations

Under the following conditions, the Verona and District Medical Services Committee agrees to pay $1000 in gold to the person who first provides to the committee the name of a physician who signs a five year contract to practice in Verona:

1. NOT eligible for the reward are:
   • Members of the Verona & District Medical Services Committee (VDMSC) or family members living in their household.
   • Members of the VDMSC Resource Team or family members living in their household
   • Employees of the Verona Medical Clinic or members living their household.
   • The successful physician applicant may not collect the reward .
2. Contest begins April 1, 2009.
3. Contestants may submit up to THREE applications per MONTH.
4. List of medical graduates or other lists of MDs are not accepted.
5. Contestants must agree to the committee's use of their name when contacting the physician(s) they put forward.
6. Contestants must agree to the use of their name and photograph for publicity purposes.
7. In the event of two contestants submitting the same name, the earlier submission will have precedence. Date and time of each submission is determined by the timestamp applied at the time of entry on the Verona Community Association website.

Any interesting tidbits about your proposed physician?
A member of the Verona District Medical Services Committee will contact your proposed physician with an invitation to reply. Followup is then the responsibility of the prospective physician. Candidates expressing interest must meet the requirements as set out by the committee and may not necessarily be chosen for the position.   I have read the rules and regulations of this contest and agree to follow them.
